Profound exercise-induce ST segment depression in a patient with Wolff-Parkinson-White syndrome and normal coronary arteriograms.

چکیده
The electrocardiogram is notoriously unreliable for the diagnosis of ischaemic heart disease in patients with Wolff-Parkinson-White syndrome. Abnormalities in the resting tracing may lead to false positive diagnoses of inferior or anterior myocardial infarction. Although ST segment depression with exercise has been reported in patients with Wolff-Parkinson-White syndrome, the depth of depression has been modest (2 mm or less).' In the present report we describe marked ST segment depression (>4 mm) during exercise in a patient with Wolff-Parkinson-White syndrome who was shown subsequently to have normal coronary anatomy by arteriography and normal myocardial function by resting and exercise radionuclide angiography.

Exercise testing in Wolff-Parkinson-White syndrome: case report with ECG and literature review.
ECG changes during exercise stress testing, such as false-positive ST-segment depression and disappearance of the delta wave, are reported in patients with the Wolff-Parkinson-White (WPW) pattern. We present a case of exercise testing in a 53-year-old man with WPW syndrome with ischemic-appearing ECG changes and normal nuclear stress perfusion study findings who was thought to be at clinically ...
متن کاملPost-paroxysmal tachycardia syndrome.
Inversion of the T wave of the electrocardiogram may persist for a long time after an episode of paroxysmal tachycardia in the absence of detectable heart disease. Mattingly (1959) has stressed the existence of "spurious heart disease" as a result of erroneous diagnosis. This may lead to unnecessary restriction of physical activity and diet, expensive diagnostic procedures and therapy, and loss...
متن کاملSubarachnoid hemorrhage mimicking ST-segment elevation myocardial infarction after return of spontaneous circulation
Electrocardiogram changes in subarachnoid hemorrhage (SAH) have been described as ST-T changes that mimic acute coronary syndrome and even acute ST-segment elevation myocardial infarction. Elevation of cardiac enzymes and abnormality of regional myocardial wall motion have been reported frequently for SAH. We report a case of an out-of-hospital cardiac arrest survivor with high suspicion of ST-...
متن کاملPathophysiology and management of patients with chest pain and normal coronary arteriograms (cardiac syndrome X).
Patients with cardiac syndrome X (CSX)—typical chest pain and electrocardiographic changes suggestive of myocardial ischemia despite normal coronary arteriograms–represent a diagnostic and therapeutic riddle. CSX is not associated with an increased mortality or an increased risk of cardiovascular events, but it often severely impairs quality of life and represents a substantial cost burden to t...
متن کاملBroken heart during treadmill exercise testing: an unusual cause of ST-segment elevation.
Treadmill exercise testing is a commonly used diagnostic test for the assessment of chest discomfort and exercise-induced arrhythmias. The presence of ST-segment elevation during exercise is considered a marker of severe ischaemia, usually secondary to a critical lesion in a proximal coronary artery. We present a novel cause of ST-segment elevation during exercise testing: "broken heart syndrom...
